Medical Case Management (MCM)

Case Management services for people living with HIV (PLWH) so they can access the healthcare and other services they need to stay healthy.

• 3 locations: NOVUS Health, Southampton Healthcare, and 3737 N. Kingshighway at Williams & Associates. 
• Includes: General Case Management, Linkage to Care, Intensive, and State Plan Personal Care. 
• PLWH living in the St. Louis, MO area with incomes below 300% of the Federal Poverty Level qualify for services. However, there are services available for individuals who do not meet case management income requirements through Ending the HIV Epidemic funding.

Early Intervention Services (EIS)

Extra support to overcome barriers to engaging in HIV care. 

• Identification & Location- links PLWH back to care.
• Intensive Support & Health Coaching- extra support for Transgender Expansive folks, justice-involved individuals, and folks who have gaps in medical care.
• Outreach- promotion of the Linkage to Care Hotline and Ryan White services.
